About this Event

CHI Memorial Bariatric and Metabolic Care offers free educational seminars for patients ages 18-69, who are interested in finding out more information about bariatric surgery including laparoscopic gastric band, sleeve and bypass. This will be a virtual live event where you will come to our office and watch our seminar and complete new patient forms, which will help get a new patient consultation more quickly. We will contact you the day before to ask the covid screening questions. You will be required to wear a mask the entire time you are in the office and our staff check your temperature upon your arrival. Please bring your insurance card and photo ID. We are limiting each session to a safe number of people to honor social distancing. We will offer multiple sessions each week so if one is full, please search for the next one. We are not allowing children in the office and we do not have room for you to bring a guest with you to the seminar, unfortunately. If you have any symptoms of illness at all or have been exposed to someone who is ill, please contact us to release your spot for someone else and you can register for another session.
